A Simple Test With a Surprising Result
The study everyone is referring to followed 1,104 active male career firefighters for about ten years. Researchers at the Harvard T.H. Chan School of Public Health measured how many push-ups each man could complete at the start, then tracked who went on to experience cardiovascular events over the following decade. The men were sorted into five groups based on their count: 0 to 10, 11 to 20, 21 to 30, 31 to 40, and more than 40.
The contrast at the two ends was striking. Compared with men who could do 10 or fewer push-ups, those who could do more than 40 had a 96 percent lower rate of cardiovascular events. Over the full follow-up, there were 37 heart-related outcomes in the group — and all but one occurred in men who had done 40 or fewer push-ups at the start. The direction of the pattern was hard to miss: more push-ups generally lined up with fewer heart problems.
The Numbers Behind the Headline
That 96 percent figure is the one that traveled, and it is worth understanding exactly where it comes from. It reflects a crude, side-by-side comparison of the highest and lowest groups — a raw rate ratio, before adjusting for other differences between the men.
When the researchers accounted for age and body weight, the picture became more measured. In the fully adjusted analysis published in JAMA Network Open, only one group — men who could do 21 to 30 push-ups — showed a statistically significant lower risk than the lowest group, with a hazard ratio of roughly 0.25. The other comparisons pointed in the same encouraging direction but did not reach statistical significance, in large part because 37 events across the whole cohort is a small number to work with. In plain terms: the trend is real and consistent, but the precise size of the benefit is less certain than the headline suggests.
How It Compared to a Treadmill Test
There was one more detail that helped the study capture attention. Within this group of men, push-up capacity was more strongly associated with future heart events than the aerobic fitness estimated from a submaximal treadmill test done at the same baseline. The researchers were careful not to oversell that point, and neither should we — a push-up count is not a clinical stress test. But it was a memorable reminder that a free, two-minute movement can sometimes carry as much signal as a far more elaborate assessment.
