Curcumin is the principal active compound in turmeric, the bright orange spice you probably already have in your kitchen. The evidence is dense enough now that curcumin has earned its place alongside omega-3 in the strong-evidence tier, though there are some practical considerations that often get missed.
In knee osteoarthritis, a recent systematic review and meta-analysis of randomized controlled trials found that curcumin supplementation significantly reduced serum C-reactive protein and tumor necrosis factor-alpha compared with placebo. Traditional turmeric used in cooking has a long history of safe use; the concern is concentrated supplements. Beyond the liver consideration, curcumin can mildly increase bleeding risk, may cause GI upset at higher doses, and can affect the metabolism of drugs cleared through the CYP3A4 enzyme system — relevant for tacrolimus, certain statins, and several chemotherapy agents. Curcumin is most worth considering if you are managing osteoarthritis, looking for an adjunct in rheumatoid arthritis or inflammatory bowel disease under specialist care, or addressing metabolic inflammation alongside lifestyle changes.
