The Coverage Gap Behind the Outbreaks
Measles outbreaks are, at their core, an immunity math problem. Protection works best when it is collective: when at least 95% of a community is vaccinated, even the people who cannot be — newborns, for instance — are shielded because the virus runs out of places to go. Slip below that line, and the math tips the other way.
That is precisely what has happened. Kindergarten MMR coverage nationwide fell to 92.5% in the 2024–2025 school year, down from about 95.2% just before the pandemic, according to the CDC's kindergarten vaccination data. That drop left roughly 286,000 kindergartners without documentation of the full MMR series in a single school year. Federal data show only 10 states and Washington, D.C., still met the 95% benchmark, with Idaho reporting the lowest kindergarten coverage at 78.5%.
National averages also hide sharp local pockets, so a state can post a healthy-looking number while individual towns sit far below it. State-by-state differences in health outcomes are a running theme, as our 2026 healthy living scorecard illustrates across other measures.
The World Health Organization has linked the rising vulnerability to several overlapping forces: disruptions to routine immunization during the COVID-19 pandemic, growing vaccine hesitancy in some communities, and limited access to health services for certain populations. Layer in increasing measles activity worldwide — which raises the odds that an unvaccinated traveler returns home infected — and the conditions for domestic outbreaks fall into place.
