This is the context that changes the picture most, and it was absent everywhere we looked.
The closest thing to a direct test of the observational claim is a trial called VitaMIND, run by researchers at the University of Exeter. It enrolled 620 adults aged 50 and over who had both mild to moderate vitamin D deficiency and early cognitive impairment — very nearly the population the Emory snapshot was observing, except that these participants were confirmed to be deficient. Half received daily vitamin D and half received placebo, for 24 months. The conclusion was that vitamin D supplementation produced no measurable improvement in cognitive outcomes, on the primary measure of executive function or on any secondary measure of cognition, function or well-being. Subgroup and per-protocol analyses came back null as well.
The Finnish Vitamin D Trial went bigger and longer, following the question all the way to dementia diagnoses. It randomized 2,492 dementia-free adults with a mean age of 68 to placebo, 1,600 IU a day, or 3,200 IU a day, for up to five years. Over a mean follow-up of 4.2 years there were 18 dementia diagnoses in the placebo group, 14 in the 1,600 IU group and 13 in the 3,200 IU group, with no statistically significant differences between the arms. Age, sex and body mass index made no difference to that result.
Then there is VITAL, the largest of the three on cognitive outcomes, whose two cognitive substudies tested 2,000 IU a day against placebo in 4,218 older adults over two to three years. The pooled difference in the annual rate of cognitive decline was 0.01, with a confidence interval spanning zero and a p-value of 0.39 — a null result, in other words, though the authors flagged a modest signal among Black participants that they said would need confirmation.
Add those together and you get 7,330 people, randomized, followed for years, with vitamin D actually administered rather than self-reported. None of the three found the benefit the headline promised. That does not make the 54-person study wrong, and it does not close the question. It does mean a small snapshot arriving after those trials is a reason for curiosity rather than a reason to change anything.
