A Reactivation, Not A New Infection
Shingles, known medically as herpes zoster, is a painful rash caused by the varicella-zoster virus, the very same virus behind chickenpox. If you had chickenpox as a child, that virus never fully left. Instead, it settled into nerve tissue near your spinal cord and brain and went dormant, sometimes for decades, as the National Institute of Neurological Disorders and Stroke explains. Shingles happens when that long-quiet virus reactivates and travels back along a nerve pathway to your skin.
That reactivation is surprisingly common. About one in three people in the United States will develop shingles in their lifetime, and roughly a million cases occur each year, according to the Centers for Disease Control and Prevention (CDC). Most people who get it will have it only once, though it is possible to experience shingles more than a single time.
You might assume this only applies to people who clearly remember having chickenpox, but more than 99 percent of Americans born before 1980 had it, even if the memory has faded. In practical terms, the vast majority of older adults carry the virus that can reactivate.
Why it wakes up in one person and not another is not fully understood, but a few patterns are clear. The risk climbs with age, especially after 50, and it rises when the immune system is weakened, whether by conditions such as certain cancers, including leukemia and lymphoma, or HIV, or by treatments like chemotherapy, radiation, and long-term steroid or immune-suppressing medications, as Mayo Clinic notes.
