Red Flags, and Why Self-Supplementing Is Trickier Than It Looks

Among the symptoms that warrant prompt evaluation: very heavy or prolonged menstrual bleeding (longer than seven days, soaking through products in under two hours, passing large clots, or bleeding between periods), blood in the stool or black, tarry stools not explained by iron supplements or medications such as bismuth, unintended weight loss, severe fatigue paired with chest tightness or shortness of breath at rest, pregnancy with persistent symptoms, or a postpartum recovery that feels off well beyond the typical window.
A second reason to involve a clinician is the supplementation question. Iron supplements are easy to buy and widely marketed, but starting them without testing first is a real and underappreciated problem. Self-supplementing can mask the cause — if a slow GI bleed is driving the deficiency, an iron pill may patch the lab number while the underlying issue continues. People with undiagnosed hereditary hemochromatosis can build iron toward toxic levels from prolonged supplementation, with consequences for the liver, heart, and pancreas. And high-dose iron causes real GI side effects — nausea, constipation, abdominal cramping — that lead many women to abandon a supplement that was not matched to their actual need in the first place.
A separate point worth knowing if a clinician does suggest iron: the dose under discussion is almost always elemental iron — not the compound number printed on the front of the bottle. The same "325 mg" on a label can mean 38 mg, 65 mg, or 106 mg of usable iron depending on which iron salt the supplement uses. A quick look at three forms common on US shelves shows the spread:

The reasonable path is straightforward: notice the pattern, bring the questions, get the panel, and let the labs and your clinician shape what comes next. Iron is one of the most treatable nutrient gaps in adult medicine, and the first step is information, not action.
